To the best of my knowledge, all statements I have given herein are true and correct. I understand that my hiring, if so, will be predicted on the truthfulness of my statement.

I understand that any misrepresentation or falsification I have made can be considered as sufficient cause for my dismissal. Furthermore, I agree and authorize the Company to:

1. Use my personal information to process my job application.
2. Share & use my personal information to affiliates and necessary third parties for my legitimate business purpose. I am assured that system securities were deployed to protect my information.

I also acknowledge and warrant that I have acquired the consent from all parties relevant to this consent and hold free and harmless and indemnify the Company from complaint suit, or damage which any party may file or claim in relation to my consent.

Hired employee who fail to submit the documentary requirements (i.e. ID of SSS, Pagibig, Philhealth etc) on time shall indemnify and hold Company free and harmless from any of penalty or liability arising from the failure of the said employee to comply therewith. Pursuant thereto, said non-complying employee shall be personally charged of the corresponding penalties imposed by the Government agencies concerned.

Finally, if hired I undertake to comply with all policies, rules and regulations issued or to be issued by the Company, which include, among others, compliance with all required communication and mobile devices system, and other related equipment or gadget for efficient interaction, productivity and business operation.
